Effectiveness of Montessori-Based Activities on Agitation Among Asian Patients with Dementia: A Systematic Review and Meta-Analysis

Abstract/Notes: OBJECTIVES: Montessori based activity are supposed to be an effective nonpharmacological intervention in the treatment of agitation in western countries. However, most studies conducted to evaluate the effectiveness of Montessori based activities on agitation in Asian patients were small sample size, as well as inconsistent outcomes, which may limit the reliability of the conclusions. The present pooled analysis, hence, was conducted to evaluate the effectiveness of the activity on agitation related with dementia in Asian patients with dementia. DESIGN: Prospective randomized clinical studies were included, of which available data was extracted. Outcomes of physical aggressive behaviors, physical nonaggressive behaviors, and verbal aggressive behaviors were pooled for the analysis by weighted mean differences. DATA SOURCES: Medline, Embase, Cochrane Library, China National Knowledge Infrastructure (CNKI), WanFang, and China Science and Technology Journal Database (VIP). ELIGIBILITY CRITERIA: Prospective, randomized, controlled clinical studies, conducted to evaluate the effectiveness of the activity on agitation related with dementia in Asian patients with dementia. DATA EXTRACTION AND SYNTHESIS: Available data including baseline characteristics and interested outcomes from the included literature were extracted independently by 2 investigators. Measuring scales including CMAI and NOSIE were adopted for the efficacy comparison between Montessori based activity and standard activity. Weighted mean difference was used for the pooled analysis. RESULTS: A total of 460 participants were included in the present meta-analysis. The pooled mean difference agitation for Montessori based activity was -3.86 (95% CI: -7.38 to -0.34, P = 0.03) comparing to standard activity. The pooled mean differences for physical aggressive behaviors, physical nonaggressive behaviors, and verbal aggressive behaviors in Montessori based activity group were -0.82 (95% CI: -1.10 to -0.55; P < 0.00001), -0.81 (95% CI: -1.68 to 0.55; P = 0.07), and 0.38 (95% CI: -0.92 to 1.68; P = 0.57). CONCLUSIONS: Montessori based activities may reduce the frequency of agitation, especially in physical aggressive behaviors comparing to standard activities in Asian patients with dementia. However, the effectiveness of Montessori based activities on reduction of subcategorized agitated behaviors including physical nonaggressive behaviors, and verbal aggressive behaviors may not be reliable as physical aggressive behaviors.

Abstract/Notes: Montessori-Based Programming (MBP) in dementia care refers to a growing body of research and practice that has developed Montessori methods to facilitate self-paced learning, independence and engagement for people living with dementia. A number of research gaps have been identified in the existing literature such as a lack of cross-cultural studies and well-powered, robustly designed outcome studies. The current study investigated the use of MBP with a focus on provision in the United Kingdom. It aimed to identify MBP implementation approaches, challenges and barriers, and research gaps.Design and MethodsA qualitative design was implemented to analyse data from in-depth, semi-structured interviews with key stakeholders (N = 8) with experience of MBP in the UK. Participants included care home management and staff, MBP trainers and independent dementia experts with a background in Montessori methods. Thematic analysis identified 4 main themes and 12 sub-themes. The study took place between April 2019 and October 2019.FindingsA framework describing knowledge and understanding of MBP in the UK, implementation considerations, challenges and barriers, evidence of outcomes and research gaps was developed to provide guidance for researchers and practitioners. Implementation considerations included using a whole-home approach and changing the culture of care through management support. Barriers to implementation included conservative attitudes to care, perceived lack of time and resources, health and safety issues, and issues of sustainability.ConclusionThe benefits of MBP in dementia care are promising but require further empirical investigation. There is a need to design, execute and publish evidence to secure the support of key stakeholders in dementia care research, policy and commissioning in the UK.

Abstract/Notes: Background: Patients with dementia experience a variety of neuropsychiatric symptoms and behavioral disturbances. The Montessori method is a type of non-pharmacological intervention to care for people with dementia. However, there are few bibliometric studies on the application of Montessori methods. We aimed to analyze the hotspots and trends of research on the application of Montessori methods to the care of dementia patients. Methods: Microsoft Office Excel, Co-Occurrence 9.9, and CiteSpace were used to analyze the articles on Montessori intervention in patients with dementia from 2000 to 2021 in China National Knowledge Infrastructure, Wanfang, China Science and Technology Journal Database, Web of Science core collection database, PubMed, and Scopus. Results: A total of 23 Chinese language publications and 113 English language publications were included. The number of English language publications was on the rise, while the number of Chinese language publications was low. There are many issuing institutions which published articles in this field, mostly concentrated in universities. English language publication sources were more than Chinese language publication sources. The hot research topic in Chinese language publications and English language publications was the care of agitated behavior of dementia patients based on the Montessori method. The psychological problems of dementia patients are likely to become a hot issue of concern for scholars in Chinese. There will be a lot of research focusing on dementia patients and their family caregivers in this field. Conclusion: The bibliometric and visualization analysis helps us understand the current research status and hotspots of Montessori intervention in dementia patients in Chinese language publications and English language publications.

Abstract/Notes: Background: To evaluate the effect of Montessori method on cognitive and behavioral function in patients with senile dementia by meta-analysis. Methods: The randomized controlled trials of the effect of Montessori method on cognitive and behavioral functions of Alzheimer’s patients in PubMed, Cochrane Library, Web of Science, Excerpt Medica Database, The Cumulative Index to Nursing & Allied Health Literature, Chinese BioMedical Literature Database, China National Knowledge Infrastructure, Weipu Information Chinese Periodical Service Platform database and Wanfang database were searched by computer, and the references of relevant literatures were traced. Two researchers independently conducted literature screening, data extraction, evaluation and inclusion in the literature, using RevMan 5.4.1 software for meta-analysis. Results: A total of 11 literatures were included and 936 patients were enrolled. Meta analysis shows that compared with routine nursing, Montessori method is helpful to improve the overall cognitive function (standardized mean difference = 1.53, 95% confidence interval (1.32, 1.73), P < 0.01), activities of daily living and reduce the incidence of adverse events (relative risk = 0.37, 95% confidence interval (0.21, 0.63), P < 0.01) in patients with Alzheimer’s disease. Conclusion: Montessori method is helpful to improve the overall cognitive function, activities of daily living and reduce the incidence of adverse events in patients with senile dementia. Affected by the included study, it still needs to be confirmed by multi center and large sample randomized controlled study.

An Analysis of Verbal Interaction in Multiage Classrooms

Abstract/Notes: The purpose of this study was to analyze the verbal interactions of children assigned to multiage classrooms. Specifically, the study investigated whether children of a particular age group interact with other children across age groups and the types of interactions each age group display. It also investigated student-to-teacher interactions and student-to-student interactions to see if interactions initiated by children are dispersed across age groups. An interaction in this study is defined as a verbal exchange between two children or between a child and the teacher. Groups studied consisted of classrooms containing children of two ages (e.g., six- and seven-year-olds) and groups containing children of three ages (e.g., eight-, nine-, and ten-year-olds). When three ages were combined in a classroom, the older children tended to dominate the conversation directed to other children in the class. Abstract/Notes: The highlights of Montessori's accomplishments are embodied in her cosmic views and organized into a Cosmic Plan of Education while she worked in the hills of Kodaikanal, India. The fruition of these cosmic views came late in her life and appeared to be synonymous with the development of her relationships with the warm and accepting Indian people. During this periodi Montessori devoted her energies to understanding how children naturally unfold into purposeful, yet interdependent individuals. Through all the professional challenges that Montessori faced in her lifetime as an educator of young children, her focal point continued to be 'the child'. This was especially true in connection with children during their transitional stage of growth from five- to seven-years of age when the acquisition of social, spiritual and cultural values are so strong. Montessori not only shared her methods and ideas with the people of India, but these people, in turn, influenced her views significantly enough to have caused her to formulate The Cosmic Plan of Education. This plan was designed especially for the transitional-age children but incapsulated within it was Montessori's own new-found East-West Cosmology. Through primary sources such as interviews, unpublished books and lectures, the dissertation gives the historical backdrop of the emergence of Montessori's cosmic views in the beginning five chapters. A description of the operation of the Cosmic Plan of Education during its conception is given in Chapter Six. The final chapters deal with how these views weathered the changes within the Indian society over the period of forty years.

Abstract/Notes: Did the work of Maria Montessori have an influence on the educational methods used in the infant and elementary school classrooms in England and Wales between 1910, when her work was first publicized in England, and 1933, when the Consultative Committee of the Board of Education published its report on Infant and Nursery Schools? To answer this principal question the following factors are considered: the effect on educational practices of the slow development of government provided education, the characteristics of the British Infant School, the social and political circumstances which were effecting education in 1910, the nature of Montessori's fame, and the official reaction to Maria Montessori and her work. For information concerning the methods actually being used in the classrooms from 1928 to 1933, the primary sources are memoranda submitted during that period to the Consultative Committee on the Primary School and to the Consultative Committee on Infant and Nursery Schools. The documents were analyzed for references to Montessori, for evidence of teachers having attended Montessori training courses, and for descriptions and advocacy of classroom methods which, according to a definition given by the Consultative Committee in its published report, reflect a Montessori influence. Eleven of thirteen Junior School Inspectors, six of thirteen Infant School Inspectors, and eleven of sixteen Heads of Infant Schools, according to the criteria used, advocate Montessori methods for more than half of the curriculum areas which they discuss. This evidence from the memoranda submitted to the Consultative Committees, combined with conclusions drawn from historical evidence, indicate that Maria Montessori did have considerable influence on the schools of England and Wales.
